We invite all our patients to attend our diabetic clinic at least annually. This gives you the chance to discuss any problems you have with your diabetes as well as receiving a thorough check-up of your circulation, blood pressure, feet (including seeing the podiatrist if necessary), and reviewing your recent blood tests.
You will be invited for a review annually. You will receive an invitation for this. You will need to book a 30 minute appointment with a Healthcare Assistant, followed by a 30 minute appointment with a nurse two weeks later.
This annual review will cover not only your Diabetes but any other chronic disease you may have (e.g. COPD or CHD). During this appointment, a personalised care plan will be created, enabling you to identify your own goals, action plans and any support you may need.
We are also part of the local scheme for digital photography of the eye to help check for any eye problems and you will be invited for screening on a regular basis.
Drugs for diabetes are now much more sophisticated than before and there are frequent new developments. Research also tells us about how best to treat your condition and what targets we should try to achieve in lowering your blood sugar and blood pressure in order to reduce your risk of a heart attack, stroke or eye and kidney problems. As well as visiting the clinic it is also important to have regular check-ups and blood tests as advised by your doctor to make sure we are keeping your diabetes as well controlled as possible.
