New Patient Registration

We welcome new patients, and would encourage all members of a family or household to register together. If you would like to join us, please ask for a registration form and health questionnaire. Alternatively you can download our registration forms and associated health questionnaires by clicking on the links below. Please print the forms off, complete and sign, and then return to us.

Please note we cannot register you without:

  • NHS Number
  • Photographic ID
  • Proof of current address

A change in legislation means that you no longer register with one doctor, but with the practice, but you are entitled to specify a preferred doctor for your care, and we would encourage you to specify a preferred doctor. We will in any case do our best to accommodate your preference at any time, whether you wish to see your usual doctor or a different doctor.

The completed forms should be returned to the surgery address or handed in. As they require signing, they cannot be submitted online at present. 

Please allow 48 hours for new registrations to be dealt with, after which time you will be able to call to book any appointments you require. Once you have been registered at the practice, you can then register with Patient Access to manage your appointments online.
